Conglutination refers to a condition where the foreskin is stuck to the glans and cannot be pulled over the glans freely or without difficulties. Penile adhesion surgery is uncomplicated. It is always preceded by an examination to determine whether the difficulty in pulling the foreskin over the glans is caused by penile adhesion or phimosis. Adhesion is resolved by penile adhesion surgery, phimosis by circumcision.

How does penile adhesion occur?

In most cases, penile adhesion is congenital. In a vast majority, this condition naturally passes away and the foreskin can be freely pulled over the glans at a very young age. However, if this is not the case and the foreskin cannot be loosely pulled over the glans, there are two causes: phimosis, i.e. narrowing of the foreskin or conglutination, i.e. penile adhesion.

"Opinions on the age when the boy's foreskin should be completely freely pulled over the glans, differ. In any case, unloosened foreskin is a complication for intimate hygiene and sexual life and it should be treated to prevent more serious problems," says MUDr. Simona Zaťkuliaková, our urologist

Freeing a stuck foreskin

If the foreskin cannot be pulled down freely because of phimosis, circumcision is carried out. It can be partial or full, depending on the particular condition, which is always determined by a specialist. If the cause is adhesion, penile adhesion surgery is carried out.

The operation is always preceded by an initial examination confirming that the cause is conglutination. Penile adhesion surgery is usually performed when waiting for a natural change of the condition does not bring the anticipated result, thus causing certain health or hygiene problems.

"The penile adhesion surgery involves releasing the foreskin from the glans, i.e. folding the foreskin to the root of the penis, releasing it using a fine probe and applying an antibiotic ointment on the prepuce to prevent re-sticking before healing and inflammation. No stitches are used in this procedure," says MUDr. Simona Zaťkuliaková.

The foreskin separation does not take more than 30 minutes and the operation is performed as an outpatient treatment in local anaesthesia for men and boys.

Recovery after penile adhesion surgery

After separation, light tension or slight pain in the treated area is normal. To relieve the pain, you can use standard painkillers. From the first days after the treatment until complete healing, the glans and the foreskin should be pulled down to the root of the penis several times a day to prevent re-sticking. The active substances in the ointment also help reduce the risk of a bacterial infection.

We recommend bathing the penis with pulled-down foreskin in a lukewarm chamomile infusion, which has antiseptic and healing effects. After washing, the penis should be gently, but carefully dried and treated with an antiseptic ointment.

There are no significant restrictions after the operation and return to normal activities and work is not limited if you feel well. The ability to urinate without difficulty should not be affected by the operation. Adult patients should observe sexual abstinence for at least 2, but preferably 3 weeks after the surgery.

Risks and possible complications connected with penile adhesion surgery

After separation of the foreskin, healing is usually trouble-free and without complications, but problems may occur in bleeding from areas where the adhesion was already very solid. The treatment area may also become increasingly sensitive or inflamed, which requires further treatment. In rare cases, the skin can stick to the glans again and the operation must be repeated or, if the problem is not satisfactorily resolved, circumcision must be carried out.

What to pay attention to

When manipulating with the foreskin, make sure that it is always pulled over the glans, not in the opposite way, ie. towards the root of the penis. This could lead to throttling or swelling of the foreskin and glans and subsequent inability to pull the foreskin back. If this situation occurs, seek an expert/urologist as soon as possible.
