Frenuloplasty surgery is recquired when the frenulum is short, thus causing problems during erection, sexual intercourse, pain or bleeding when the glans skin is torn. It is also one of the possible ways to deal with premature ejaculation.

What does frenuloplasty solve?

The frenulum breve is located under the foreskin and in normal condition, it helps to pull the foreskin over the glans. When the foreskin is shortened, the pull is tight, which leads to great difficulties in the man's intimate life. Erection and sexual intercourse with a shortened foreskin can be very unpleasant or even painful. The area surrounding the frenulum is often torn and accompanied with bleeding. This is painful and can lead to an infection or inflammation in the wound area.

These problems can be resolved for good by frenuloplasty. In some cases, the men suffer not only from short frenulum, but also from narrow foreskin. This problem is removed by frenuloplasty combined with circumcision.

When should you have a frenuloplasty?

Frenulotomy is intended for men having trouble with short frenulum during sexual intercourse or suffering from premature ejaculation due to its shortening. During erection and sexual intercourse, the frenulum becomes more stressed and irritated. As the sensitive nerve endings are most concentrated in its area, it can lead to premature ejaculation. The actual frenuloplasty need not guarantee prolonged sexual intercourse, but it may happen to a certain number of men. In these cases, frenuloplasty is often combined with circumcision, much like in the case of problems connected with narrowed foreskin (phimosis).

Before the frenuloplasty

The surgery is always preceded by a consultation with a specialist who assesses the condition and determines whether frenuloplasty should be combined with circumcision (in case of a narrow foreskin). During the consultation, the doctor explains the procedure and answers all questions connected with the operation.

Up to 14 days before the surgery, standard pre-operative examinations should be carried out, namely blood and urine tests to ensure that the client is in a good overall health condition, which is a prerequisite for the surgery.

Frenuloplasty procedure

Cutting the frenulum is not a difficult procedure. At our clinic, it is performed as an outpatient treatment under local anaesthesia. It takes about 10 minutes.

"During frenulotomy, we first cut the frenulum crosswise with a coagulation catheter and then reconstruct it lengthwise by stitches using fine absorbable suture, thus prolonging the frenulum. After healing, the frenulum can perform its normal function, i.e. pull the foreskin. Finally, we locally apply an antibiotic ointment on the wound. The surgery is not time-consuming and the patient can leave for home immediately afterwards", as MUDr. Simona Zatkuliaková, the eveclinic urologist and surgeon explains the process of the surgery.

Recovery and aftercare after frenuloplasty procedure

During the first days, the penis may hurt slightly and swell. To improve your comfort, you can take common painkillers. The swelling and pain will subside in a few days. Apply an antibiotic ointment to the scar to minimize a risk of infection and prevent sticking to the underwear. We recommended wearing loose, light underwear the first 2 to 3 days after frenuloplasty.

From the second day, we recommend bathing the penis in a lukewarm solution of chamomile. After the bath, dry the scar gently, but carefully and apply an antibiotic ointment. We also recommend pulling the foreskin down to the root of the penis from the second to third day until complete healing. This will prevent scarring the frenulum and its re-shortening.

Make sure to keep a more relaxed routine in the first seven days after frenuloplasty. You can perform common daily activities and light office work without restriction. However, you must avoid physically demanding work, lift or carry heavy loads and all sports activities. (Children can normally play, but the games should not involve excessive physical effort.) Swimming in pools or natural pools or visiting saunas, solariums or Turkish baths is completely inappropriate.

Sex and urination after frenuloplasty

Sexual abstinence should be observed until complete healing, which is usually 3 to 4 weeks after the surgery. If instructions for frenuloplasty are followed, full functionality of the penis returns within 14 days. The operation should have no effect on urination.

Postoperative examination

The client undergoes a postoperative examination 7 to 10 days after frenuloplasty at Eveclinic in Grösslingova Street 7, Bratislava.

Possible risks and complications after frenuloplasty

Frequent complication following frenuloplasty include swelling of the penis, which lasts for several days, infection in the incision area and bleeding from the wound. Less frequent (< 2 %) complications include scarring, changed sensitivity of the penis, imperfect satisfaction with the cosmetic effect or potential need of earlier circumcision if overall improvement of the difficulty is not achieved.
